Established in North Lanarkshire since 1998, we have a track record of supporting a wide variety of people to meet a wide variety of outcomes. We are committed to a co-production approach working with people and their families in supporting them to plan and achieve their personal outcomes.
Personal outcomes we have supported people to achieve have ranged through having a clean and tidy home, playing football, completing a college course, going on holiday, getting a job, making friends, building relationships, having fun and enjoying life!
Through supporting people to achieve their personal outcomes we support people to have a better physical health and mental wellbeing, to be better connected to their communities and be as independent as they can be.
We are able to support people to develop their own plan about how they want to use their individual budget to meet their needs and this will ensure we meet the 7 criteria for a good support plan. Or we can offer guidance and support so that people and their families can do this themselves.
Our service can provide supports through different service options, some of these are ‘off the shelf’ others require a bespoke service.
Our Chryston Connect service offers support to people who have severe and enduring mental health needs, we support people to make connections, build confidence and increase skills.
In North Lanarkshire we have a Shared Lives service; this service offers an alternative model of support to people who have individual budgets. Through this service we will be able to offer Short Breaks, Day Options and Kinship Care. This service links people and communities through a flexible, informal, approach to support.
We have both organisational and service specific induction training in place for all staff.
